This form is required and must be filled out completely with all mapping submissions. Pursuant to the New Mexico 811 (NM811) membership manual, members are responsible for notifying NM811 in writing of any changes in its coverage area thirty (30) days in advance of any purchases, sales, or the retirement of underground facility assets affecting their service areas. The information requested is vital in processing your submission correctly and in a timely manner. Incomplete forms will be returned to members for any additional clarifications or information necessary to process their request.

Deletes are considered to be the removal of all or any portion of a currently registered asset database file associated with an existing member code. Delete requests require a brief description or reason in the “Additional Information” box above prior to NM811 processing the request and thereby avoiding any unnecessary processing delays. The removal of Sold Assets requires written notification by an authorized company representative should be sent to our help desk at membership@nm811.org
**NOTE: It is the responsibility of both the seller and purchaser of existing underground facilities to coordinate the transfer of such assets and to submit the appropriate Membership Mapping Update or New Membership application reflecting their respective transactions in a timely manner so as to ensure the continual coverage and protection of all such assets.
Helpful Hints: • Mapping update requests are placed into live production every Tuesday and Friday and may experience delays depending on the complexity and/or volume of updates received or the receipt of outstanding information required to complete a members request. • It is a Members responsibility to review file attachments immediately to ensure accuracy of update(s). Formats accepted: shapefiles (preferred), lat/long coordinates, tab files, Google My Maps, and .kml/.kmz files (you can easily get .kml/ .kmz files using Google Earth). Paper maps must include street names and CAD files must provide the projection type format but should preferably be set to WGS84.
